Cox 2 inhibitors Flashcards
What is the benefit of COX-2 inhibitors?
They only effect Cox-2 enzymes. Cox-1 enzymes have an important role in inflammation. NSAID’s block both COX-1 and COX-2 enzymes
What are some COX-2 inhibitors?
Celebrex, Vioxx, Bextra (valdecoxib
What is celecoxib (celebrex) indicated for?
Treatment of adult RA and osteoarthritis
What are the side effects of COX-2 inhibitors?
Fluid retention and edema
What are the adverse effects of COX-2 inhibitors?
Cardiovascular incidents (vioxx), potential liver dysfunction, GI bleeding, allergy
What drugs do COX-2 inhibitors interact with?
Diflucan, LIthium, ACE inhibitors
What do glucocorticosteroids affect?
Stimulate formation of glucose and promote storage of glucose as glycogen
promote the catabolism of proteins to form amino acids which travel to liver and are converted to glucose and glycogen
Lipid metabolism-lipolysis of triglycerides
What is the method of action for corticosteroids?
Control the rate of cellular synthesis of proteins
What type of affects do corticosteroids have on the body?
Anti-inflammatory and anti-autoimmune
What is bridge therapy?
short term therapy used while DMARD’s become effective
What are the adverse effects of corticosteroids?
Acute adrenal insufficiency
Immunosupression
Mental status changes (steroid psychosis, steroid buzz)
Occular effects (cataracts, increased ocular pressure)
Cushingoid state (moon face, buffalo hump)
Myopathy
Fluid and electrolyte disturbances
Hyperglycemia and glycosuria
GI issues
Osteoporosis and vertebral compression fractures
What reduces the risk of systemic side effects associated with oral corticosteroids?
Intra-articular joint injections
