Osteochondral fragmentation and OCD Flashcards
Decsribe how OCD and OCF are different
Both have osteochondral fragments in joints
OCD is the end result of osteochondrosis - developmental disease
OCF is traumatic / degenerative - part of osteoarthritis
What are the strong predilection site of OCD
locations where bones slide over each other:
Stifle (lateral trochlear ridge femur)
Hock (distal intermediate ridge of tibia)
How do we diagnose OCD
usually by radiography
What does DIRT mean
OCD of distal intermediate ridge of tibia (hock)
what is OCF
Small piece on periarticular bone fractures off in a traumatic manner
can be called chip fractures
How can OCF occur
as part of OA
OR
Following direct trauma
OR
As an avulsion associated with a ligament insertion
List 2 ways that OA can cause OCF
periarticular osteophytes form then fracture off
subchondral bone disease creates weakening - bone breaks away
List 3 predilection sites of OCF
carpus
fetlock
coffin joint
when do clinical signs of OCF appear
straight away
later when the fragment contributes for further OA development
never
what can we see with OCF
Mild to moderate lameness – worse after flexion
Joint effusions & heat
when would we do conservative mangement of OCD nad OCF
if no clinical signs and not for sale
describe medical treatment of OCD and OCF
IA medications, NSAIDs
Temporary improvement
Likely to lead to OA in future due to ongoing synovitis
how do we surgically treat OCD and OCF
arthroscopic fragment removal
Describe Arthroscopic fragment removal
Fragments removed
Necrotic underlying bone debrided
Fibrillated cartilage trimmed
what is the prognosis of Arthroscopic fragment removal for OCD and OCF
excellent prognosis
physiotherapy important
short recovery
often return to full soundness
