Osteoarthritis Flashcards
The underlying imbalance in osteoarthiritis
Cartilage breakdown and chondrocyte regeneration
Cytokines that cause inflammation in osteoarthritis are released by
Macrophages
Protease that breaks down cartilage are released by
Protease
How do cytokines lead to the formation of osteophytes
Stimulate osteocytes —> recruit osteoblasts to repair bones
3 conditions that MUST be met to confirm a diagnosis of osteoarthritis
- > 45
- activity-related pain
- no morning stiffness/stiffness lasting <30 mins
Osteoarthritic changes on XR (LOSS)
- loss of joint space
- osteophytes
- suchondral space sclerosis
- suchondral space cysts
Exercise makes oesteoarthritic pain
Worse
Is there morning stiffness in osteoarthritis
No (or<30mins)
Impact on joint
- pain, stiffness
- deformity, instability
Heberden’s nodes
DIPJs
Bouchard’s nodes
PIPJs
Saddle joint
CMPJ (prone to damage)
1st step of analgesia
PO paracetamol + topical NSAIDs
2nd step of analgesia
Add PO NSAIDs & consider PPI
3rd (&cautious) step of analgesia
Opiates
