Denosumab
First potential side effect of bisphosphonate use
Osteonecrosis of the jaw
2) atypical femur fracture
MOS bisphosphonate a
Impair osteoclastic function and reduce osteoclat at number by inducing apoptosis
2 SERMS approved for prevention of osteoporosis and MOA
Tamoxifen and raloxifene
Deceased bone turnover
Potent bisohosphonate with unique administration regimen IV annually
Zolendronic acid
Fractures in these bones that occur in the absence of major trauma is sufficient to diagnose osteoporosis
Regardless of BMD: - spine - hip In the presence of low BMD:& - pelvis - proximal humerus - wrist
Indications for BMD
Women >65, men age 70
Younger post menopausal women
Adults with fracture after age 50
Adults with a condition (RA) or taking meds (GC >=5 mg prednisone for more than 3 mos)
Men 50-69 with risk factors for osteoporosis
Height loss indication for BMD
1-1.5 in
WHO definition of osteoporosis
Bone density falls 2.5 standard deviations below the mean for young healthy adults of the same sex— also referred as T-score of -2.5
LOW BONE DENSITY- if T score
Standard for measuring bone density
DXA
Measure lumbar spine
Femoral neck
Total hip
Fractures in this site that occur in the absence of trauma regardless of BMD may be diagnosed as osteoporosis
Hip
Spine
Fractures of these sites in the setting of low BMD may be diagnosed as OSTEOPOROSIS
Pelvis
Proximal humerus
Wrist
(In the absence of major trauma)
DD: low urine calcium
Osteomalacia
Malnutrition
Marker for bone resorption
C-telopeptide
SERM with reduction in breast Ca effect
Raloxifene
