What is Tinea Pedis

A

Fungal infection of the foot also known as athletes foot which is commonly caused by dermatophytes

What are the different types of Tinea Pedis

A

So there are 3 main types of tinea pedis:
Interdigitial:
* commonly occurs between 4th and 5th digit
* can have visible maceration between the toes but not always
* patient will describe itching or burning symptomns
* May spread medially to other areas of the foot

Moccasin:
* Covers the plantar aspect and the sides of the foot
* Most commonly caused by t-rubrum
* displays dermacted scaling
* tiny vesicles or pustules can occur

Vesicobullus:
* Mainly on the arches and soles of the feet
* An inflamatory variant with erythamatous base, pustules and blisters

What assesments would you do to diagnose this condition

A

Can be diagnosed by the clinical presentation if suspected fungal infection ask the following:
* nature, site and duration of any symptomns
* any co morbidities
* any previous treatments
* Any infected family members

Take skin smaples for fungal microsocpy:
* scrape the skin from the edge of the affected area
* collect 5mm2 of skin flakes to send off for mycroscopy

How would you treat manage the condition

A

provide Self care advice on management:
* Avoid occlusive footwear
* change footwear every 2-3 days where possible
* Avoid scratching the affected skin as this could spread the infection to other areas
* wear cotton absorbent socks
* wear protective footwear when using communal spaces to avoid cross contamintsion

MANAGEMENT would depend on the organism and the charecteristics of the patient

Treatment options:
In MILDcases a topical solution would be first line treatment option
* so a terbinafine, econazole or miconazole cream
* If theres any associated inflamattion a corticosteriod could be given aswell such as 1% hydrocortisone cream

In SEVERE cases an oral systmeic would be given as first line treatment option:
* Oral terbinafine, itraconazole or griseofulvin

A

Immunosupression
Diabetes
Athletes or active individual
