What is Team Nursing?
Groups of patients are assigned to a team headed by a team leader
- usually an RN
What does team nursing success rely on?
Good communication between all members
Benefits of Team Nursing?
Cost effective
Team makes decisions together
Improved patient satisfaction
Disadvantages of Team Nursing
Relies on a good, skilled leader
Need skilled staff as well
Chain of command for Team Nursing
Charge > Team leader > nursing staff >patients
What is Modular Nursing?
Similar to team nursing but uses a smaller team. Pairs pro nurses with other staff (CNA, PCT) to deliver care to groups of patients
- (nurse gets their very own cna/pct essentially).
Benefits of Modular Nursing
Better communication
Runs smoother
What is Primary Nursing/relationship based nursing?
What is its new name?
Where would this work?
Nurse plans and directs care of patient over 24 hour period. This doesn’t mean they’re there for the entire time though.
New name is Relationship-based practice.
- RN managers and coordinates patients care and discharge plan
Longterm acute care, rehab. Definitely not acute care.
Benefits of Primary Nursing?
Continuity of care
More personalized
Disadvantages of Primary Nursing?
Wouldn’t work in acute care
Need experienced nurses
What does Case Management involve? (3)
Collaboration - in order to better the patient
Coordination - coordinating care through the episode of illness. And figuring out the next steps for them
Focus - Uses critical pathways, action plans, maps, etc to focus on the plan to make sure everything is moving along
Takes into account cost but also quality of care
How is the mode of care chosen ?
Base it off patient acuity
- not just economics.
Knowledge and skill of the staff
- geographically, those skills may not be available
Consider the desires of the nurses
- not every nurse wants a lot of autonomy

Clinical nurse-leaders
Nurses who embrace patient and family-centered care
What is a nurse navigator?
Works in targeted clinical settings
- breast cancer nurse navigator may work with the pt from time they were diagnosed throughout their care
- continuity of care, a guide, resource
What is a clinical nurse-leader?
Clinical experts who lead other team members (expert bedside nurse)
