Atracurium (intermed)
Intubate: 0.5mg/kg Maintenance: 0.08-0.1mg/kg q 20-45 min
Cisatracurium(intermed)
Intubate: 0.2mg/kg Maintenance: 0.03mg/kg q 40-60 min
Rocuronium (short/intermed)
Range: 0.6-1.2mg/kg IV RSI: 0.9-1.2mg/kg IV Maintenance: 0.1-0.2mg/kg IV (0.3-1.2mg/kg from short to long)
Vecuronium (intermed)
Intubate: 0.1mg/kg Maintenance: 0.01-0.015mg/kg q 25-45 minutes RSI: 0.3mg/kg
Pancuronium (long)
Intubate: 0.1mg/kg Maintenance: 0.01-0.015 mg/kg q 40-60 minutes Cont: 1-1.7mcg/kg/min (decrease in elderly)
Mivacurium (short)
Intubate: 0.2mg/kg IV Maintenance: 0.01-0.1mg/kg q 5-15 minutes
d-Tubocurarine (long)
0.5mg/kg IV maintenance: 0.05-0.1mg/kg IV
Doxacurium (long)
Intubate: 0.05mg/kg IV maintenance: 0.01-0.025 mg/kg q 45 min(decrease in elderly)
Pipercuronium (long)
Intubate: 0.1mg/kg Maintenance: 0.01-0.04mg/kg IV q 45-120 min
Morphine (long DOA-3 hours–prototype)
2.5-15mg IV pre-op and post-op
Meperidine (1/10 potent as morphine)
50-150 mg IM 5mg q5 min IV 12.5mg IV for shivering
Fentanyl (100x more potent than morphine)
25-100mcg premed/prn 1-2mcg/kg analgesia 12.5-100mcg intra-op
Sufentanyl (10x more potent than fentanyl, 1000x more potent than morphine)
0.1-1mcg/kg bolus 1-2mcg/kg for minor, 2-8 for moderate, 8-50 for sole anesthetic
Alfentanyl (1/5 potency of fentanyl, 10-20x more than morphine)
10-20mcg/kg
Remifentanyl (same potency as fentanyl, 100x more than morphine)
0.5-1mcg/kg over 60-90sec
Hydromorhpone (5x more potent than morhpine)

Induction 0.5-2mg/kg (>2 causes delirium) Sedation 0.2-0.5mg/kg Infusion 1-2mg/kg/hr
Etomidate

Propofol

Atropine
0.01mg/kg for reversal with Endrophonium 0.4-1mg Bradycardia 1mg q 3-5 minutes for asystole
Glycopyrolate

Scopalamine
0.3-0.6mg IV q4-6 hr
Pyridostigmine

Endrophonium
0.5-1mg/kg with Atropine 0.01mg/kg

Epinephrine
1mg q 3-5minutes for cardiac arrest 1-2mcg/min beta 2 4-5mcg/min beta 1 10-20mcg/min alpha and beta 2-8mcg/kg for low BP
Norepinephrine
4-16mcg/min (or 0.01-0.1mcg/kg/min)
Dopamine
0.5-2mcg/kg/min low 2-5 medium >10 is large
Dobutamine
2-10mcg/kg/min 5 is weak alpha 1
Vasopressin
0.04 U/min for sepsis 40U push for cardiac arrest 20U IV for esophageal varices
Isopreterenol
0.5-10mcg/min
Ephedrine
5-25mg IV
Phenylephrine
50-200mcg/kg IV push maintenance 20-50mcg/min double dilute to 100mcg/cc
Ampicillin
2g IV 30 min preop
Cefazolin
1-2g IV 30 min preop
Gentamycin
60-120mg IV (>9mcg/ml is toxic)
Clindamycin
600mg IV preop
Vancomycin
1g in 250ml over an hour
Metronidazole
500mg over 10-20 minutes preop
