Foraminal Compression Test
Sparely’s Maneuver
• Asses cervical spine function. Narrowing intervertebral disc space
• Laterally flex to the unaffected Side
• Press straight down
• Laterally Flex affected side
• Press Straight down
• Positive Test: Local pain may indicate foraminal encroachment
without nerve root pressure or apophyseal capsulitis
• Radicular pain along dermatome may - pressure on a nerve root
• Possible pain in shoulder from facet joints
• Neck pain only can indicate facet pathology
• Differential diagnosis – Intervertebral Foramen Encroachment (IVFE), nerve root lesion, disc herniation, facet pathology, stenosis
Distraction test
only used for patients with nerve symptoms
Grasp beneath the mastoid processes
• Press up on the patient’s head
• Removes the weight of the patient’s head on the neck
• Tractions the cervical spine
• Hand under chin and occiput
• Positive Test:
• Local pain increases - muscle strain, spasm, ligamentous sprain, or
facet capsulitis
• Relief of radicular pain - foraminal encroachment or a disc defect
• Differential diagnosis
• IVFE, nerve root lesion, disc herniation, facet pathology, stenosis

AROM = Muscle strain/sprain PROM=ligament sprain
• Procedure: Patient seated
• Put the cervical spine through resisted range of motion,
then through passive range of motion
• Positive Test:
• Pain during resisted range of motion or isometric muscle contraction signifies muscle strain
• Pain during passive range of motion may indicate a sprain of any of the cervical ligaments
• Structures Affected: Cervical spinal muscles and/or cervical spinal ligaments
Shoulder Depression Test
Laterally flex patients head while applying downward pressure on opposite shoulder
• Positive Test:
• +Local pain on the side being tested
• shortening of the muscles, muscular adhesions, muscle spasm, or ligamentous injury
• +Radicular pain - compression of the neurovascular bundle or thoracic outlet syndrome
• +Pain on the opposite - decreased foraminal space, facet pathology, or disc defect
Jackson’s
compression with rotation to left and right
