Oestrogen Flashcards
Most potent and main estrogen in ovary
Estradiol
Where is estriol formed
Liver
Where is estrone formed
Fat cells
Synthetic steroidal estrogen
Ethynyl estradiol
Tibolone
Synthetic non steroidal estrogen
Diethylstilbestrol
Dienestrol
Does estrogen have negative feedback on both hypothalamus and anterior pituitary
No only anterior pituitary
How many types of oestrogen receptors present in body
2
=ERalpha
=ERbeta
ER alpha present in
Ovary
Uterus
Vagina
Breast
Hypothalamus
Blood vessels
ER beta present in
Ovary
Prostate
Why estrogen not effective orally
High fast pass metabolism
Enterohepatic cycle of estrogen
All 3 natural forms undergo glucose idle and sulaate conjugation
Excreted in bile
In intestines they undergo deconjugation by bacterial flora
Then reabsorbed to complete the cycle
Therapeutic uses of estrogen
- delayed puberty in girls (due to turner syndrome, hypopituitarysm)
- Oral Contraceptives along with progestin (to reduce risk of endometrial carcinoma)
- Dysmenorrhea (suppress ovulation and anovulatory cycles are painless)
- Dysfunctional uterine bleeding (if estrogen withdrawal is the cause)
- Post menopausal hormone replacement therapy(to alleviate vasomotor symptoms,hot flushes,genital atrophy,osteoporosis,sleep disturbances,night sweats)
- Senile vaginitis(topical use)
- Prostatic carcinoma(by suppressing androgen production)
Advantage of tibolone
- Has estrogenic, progesterogenic, and weak androgenic activity
- Can be used in hormone replacement therapy without cyclic progesterone
- Does not cause endometrial proliferation
Adverse effects of oestrogen
- Water retention,Edema
- Weight gain
- Increased gall stones,liver diseases
- Thromboembolism
- Increased endometrial and breast cancer
- Nausea , vomiting
Which estrogen dose is lower :HRT or oral contraceptives
HRT(one -fifth)that of contraceptives
So low side effects
