OD17 Flashcards
3 potential life-threatening complications of surgery?
• Airway obstruction
o Lugwig’s angina – swelling in FOM – PUS
• Haemorrhage (exsanguination) – can result in airway obstruction
• Infection - SEPSIS
A risk of lingual swelling, that is rare in buccal swellings
Airway obstruction
Describe a odontogenic swelling 0-3 days in.
Pus breaks through cortices
Acute inflammation
Mild erythema
Describe a odontogenic swelling 3-7 days?
Cellulitis
Hard erythematous swelling
Very painful
Drained = serosanguineous fluid with flecks of pu
Describe a odontogenic swelling After 5 days?
Potential for abscess formation
4 potential symptoms of SIRS – systemaric inflammatory response syndrome
Tachycardia- >90BPM
Tachyponea- >20RR
Pyrexia- >38.3 or <36
WCC- <4 or >12
Other signs of potential sepsis?
Purpuric rash No passage of urine in >18 hours Confuses state Only responds to pain. Trap. nip> Lactate >2
Treatment of suspected sepsis?
Broad spectrum antibiotics- 1g amoxicillin
Fluids
Oxygen
Lactate
What is the difference between bacteraemia and septicaemia.
• Bacteraemia Presence of BACTERIA in blood stream Managed by competent immune system • Septicaemia Presence of numerous ACTIVELY DIVIDING BACTERIA in the blood Produces a systemic response - SIRs Tachycardia/ tachypnoea/ raised temperature
4 main principles for treatment of infection?
- Remove cause- XLA/ access
- Institute drainage-
- Prevent spread- antibiotics
- Restore function
What causes hot potato voice?
Significant oedema around oropharynx
