Occlusal Problems in the Mixed Dentition Flashcards
what is crown root dilaceration?
bend between crown and root
caused by trauma - displacement of unerupted crown and root continues growth in different direction
why might a crown root dilaceration not be removed?
maintain alveolar bone
no need for child to wear denture
what are the 2 types of supernumerary tooth?
supplemental - identical copy of adjacent tooth, 2’s, premolars, 8’s
conical/tuberculate - contain tooth material but do not look like teeth
in what dentition should a supernumerary be removed?
deciduous - to avoid ortho
if teeth are not erupting in a normal patter what could be the cause?
supernumerarys can impede normal eruption
where are supernumeraries most common?
if a supernumerary occurs in the maxillary midline what is it called?
premaxilla
mesiodens
what teeth are commonly peg shaped?
what can they be associated with?
what problems can they cause?
lateral incisors
absent contra
spacing and aesthetic problems
what is there an increased risk of with peg shaped teeth?
ectopic canines
what is hypodontia?
the absence of one or more teeth
can be hereditary
what teeth are commonly affected by hypodontia?
what can they cause problems with?
U2’s and L5’s
spacing and aesthetics
is hypodontia more common in deciduous or permanent dentition?
permanent
what does hypodontia commonly present with?
delayed exfoliation of deciduous
delayed eruption of permanent
what are infraoccluded teeth?
teeth appear to have shrunk into the gingivae
what are neonatal teeth?
deciduous teeth that erupt at birth
can cause feeding problems
if v mobile - extract - bc uncomplete root formation
what is an eruption cyst?
blue mucosa overlying unerupted teeth
common with E’s and 6’s
asymptomatic
resolves as tooth erupts
what are impacted teeth?
deciduous/permanent teeth that fail to erupt fully
can be partly erupted
what can impacted teeth be due to?
obstruction of supernumerary
failure of eruption
insufficient space
ectopic teeth - growing or forming in wrong position
if 6’s become impacted what can happen to the E’s?
resorption and caries
why does infraocclusion occur?
no perm tooth underneath
ankylosis
if an infraoccluded tooth is left what can happen?
tooth next door tips over it
why might a deciduous tooth be retained?
missing permanent successor or ankylosis
if a deciduous tooth has ankylosed why does it need extraction?
could affect permanent successor - leave in situ if no perm successor
why might you lose a deciduous tooth early?
caries/trauma
balancing/compensating extractions
serial extractions
what can an avulsion of the deciduous incisors cause?
result in central line shift in incisors
what can delayed eruption of a deciduous tooth cause?
permanent successor may be delayed in eruption bc fibrous mucosa
what can intrusion of deciduous incisors cause?
deflection of permanent successor
what are balancing extractions?
tooth on one side XLA the other. done if concerned about centre line shift
most commonly C’s and D’s cause
what are compensating extractions?
tooth XLA in one arch remove in the other to stop over eruption and occlusal interference’s
what are serial extractions?
Kjellgren 1948
planned sequence of extraction to relieve incisor crowding in mixed dentition
why may 6’s be extracted?
Caries/MIH
want to relieve incisor crowding
when should 6’s be extracted?
when all permanent teeth have come in
age 9-10 if want 7’s to erupt well
what are crossbites?
can affect any tooth - molars/incisors
unilateral/bilateral
can cause displacement and tooth wear
what can treat crossbites?
appliances
what can thumb sucking cause?
proclined upper anteriors
retroclined lower incisors
reduced overbite and anterior open bite
buccal segment crossbites
how to treat thumb sucking?
deterrent devices
plaster on finger
encouragement
nail varnish
what percent of 6 year olds have a median diastema?
98%
when does a median diastema occur?
small teeth/large jaws missing lateral incisors midline supernumerary prominent frenulum proclined upper incisors
if under what size in the deciduous dentition can you leave a diastema?
where do E’s commonly impact?
distal of D’s
why might central incisors be impacted?
supernumeraries
crown root dilaceration
trauma
premature loss of deciduous
when should you be able to palpate the canines?
8-10 years
if 3’s are impacted what can be done?
extract c’s to allow eruption
if you cant palpate the 3’s at 10 years do what?
DPT
ensure no cyst has developed
