Materials for taking a temperature
thermometer lubricant timer cotton disinfectant
How to take a temperature
- Clean thermometer with cotton and disinfectant
- Shake down to 95 degrees
- Lubricate with lube on cotton to prevent contamination
- Insert with gentle pressure and slight rotation
- Remove after 2-3 minutes by pulling straight out
- Wipe clean, read by turning (holding at eye level, use white as background, read the end of the mercury to nearest tenth of a degree)
- Clean again with disinfectant and replace
- record temp in chart

Notes about obtaining a pulse rate
measured by palpation of artery; may not get accurate value, but can get info for pulse deficits (diff between heart rate and pulse) and increases or decreases in blood pressure

index finger over femoral artery, count the number of pulsations in 60 seconds
