Amniotic fluid embolism
A
Rare catastrophic emergency with sudden maternal collapse, hypoxia, shock and coagulopathy
Fetal tissue/amniotic fluid enters maternal circulation
Phase 1 - vasoactive production leading to sudden, severe pulm HTN and right heart failure, hypotension and hypoxia
Phase 2 - LVF, pulm oedema, capillary leak, DIC, uterine Antony, haemorrhage
RF: Advanced age, placental pathology, induction labour, operative delivery, uterine rupture, intrauterine death, cervical lacerations
Mx: CVS support, fetal delivery, MOH protocol and DIC suppport
