Observations and Malingering Flashcards
What is the patient position for minor’s sign?
seated
What observation is this?
-patient must push off when rising from chair, affected leg is bent and balancing on healthy
-patient climbs legs and torso to stand straight
minor’s sign
What a positive sign for minor’s sign? What does this indicate?
slowed motion due to pain indicates sciatica or LBP
What is the patient position for amoss’ sign?
supine or side lying
What observation is this?
-recumbent patient places hands for behind their body to assist in rising to seated position
amoss’ sign
What is the positive sign for amoss’ sign? What does this indicate?
stiffness or lack of motion or localized pain in the thoracolumbar region indicates AS, severe sprain/strain, or disc syndrome
What is the patient position for beevor’s sign?
supine
What observation is this?
-patient actively attempts partial sit up while doctor observes exposed umbilicus
beevor’s sign
What is a positive sign of beevor’s sign? What does this indicate?
umbilicus will move away to one side
indications:
-lesion at T7-T11
-characteristic of ALS or muscular dystrophy
What is the patient position for rust sign?
standing, seated, or supine
What observation is this?
-patient with a serve injury to the upper cervical spine grasps head with both hands to support the weight of the head on the cervical spine
-removal of this support cannot be tolerated
-if pt is supine, they will support head while attempting to rise
rust sign
What is a positive sign of rust sign? What does this indicate?
patient guards movements and stabilizes the head with slight traction to reduce the pain
indications: gross instability of the upper cervical spine as a result of:
-fracture
-dislocation
-severe muscle sprain
-ligamentous instability
-posterior disc defect
What is the patient position for burn’s bench test?
kneeling
What test is this?
-pt is kneeling on bench of table approx 18 in above floor
-doctor stabilizes pt leg by holding heels down and asks them to bend at waist and touch floor
burn’s bench test
What is a positive sign of burn’s bench test? What does this indicate?
inability to perform indicates malingering
What is the patient position for hoover’s sign?
supine
What test is this?
-doctor places hand under each heel and asks pt to attempt to raise affected limb
hoover’s sign
What is a positive sign for hoover’s sign? What does this indicate?
very little or no downward pressure from heel on unaffected side indicates malingering
note: it is normal to feel pressure from heel of unaffected side to show pt is making an effort
What test is this?
-pt indicates point of pain and doctor marks it
-pt is distracted with other tests away from site
-doctor later asks pt about pain and asks them to indicate site again
magnuson’s test
What is a positive sign of magnuson’s test? What does this indicate?
significant change in location of pain indicates malingering
What test is this?
-pt’s resting pulse rate is established before test
-doctor applies mechanical pressure to point of complaint while monitoring pulse rate
mannkopf’s sign
What is a positive sign of mannkopf’s sign? What does this indicate?
increase in pulse of more than 10bpm indicates NOT malingering
