A
  • previous pregnancies and deliveries (date, gestation, mode of labour and delivery, birth weights and health of children)
  • any previous problems in/after pregnancy and deliveries
  • any other problems in this pregnancy
  • same partner in this pregnancy
  • mental health problems/ traumatic birth?
  • was this pregnancy planned?
  • previous miscarriages, terminations, still birth?

  • N&V (hyperemesis gravidarum)
  • abdominal pain (imaging/ labour)
  • vaginal bleeding/discharge/leaking of waters (fresh red blood, clots, tissue, foul smelling, itchy)
  • dysuria/ urinary frequency (UTI)
  • fatigue (anaemia?)
  • headache, visual changes, swelling (pre-eclampsia)
  • systemic symptoms (fever, malaise, rigors, SOB, itching hands and feet)
  • fetal movements (if >20/40)
  • do we know rhesus status?
