Factors affecting placental transfer of drugs

A

Size: >1000 Da do not cross readily (insulin, heparin, protamine)
Lipid solubility: highly ionized don’t readily cross (succ, NDNMBDs), lipophilic do (fentanyl)
Protein binding: mother has 50% more plasma proteins compared to fetus and will maintain better (bupi, ropi)
Maternal drug concentration: higher in mom = higher rate of transfer

A

Increases significantly throughout pregnancy due to increased HR (1st trim) and SV (trim 2/3, 50% increase in CO from baseline)
Onset of labor: 110% above prepregnant state d/t SV
Immediately after delivery: SV increases d/t loss of IVC compression, >150% above prepregnant state
1 hr postpartum: 100% above pre-pregnancy
2 days postpartum: 10% above pre-preg
2-24w postpartum: return to prepregnant state
