A
-develops as a result of decreased intake of nutrients, increased nutrient requirements, and complications due to the disease process

Nutritional risk assessment
A
- are time-sensitive
- require screening for actual and potential nutritional alterations
- determine the presence of physical limitations and psychosocial factors
- may result in consultations with a registered dietitian (RD) or a speech – language pathologist (SLP)

A
- patient personal care is a primary registered nurse (RN) responsibility at prevents complication
- regular nutritional screening and assessment result in early detection and improvement of outcomes for for high-risk and malnourished patients
- failure to identify and refer malnourished patients for evaluation can result in adverse clinical outcomes

Patient - centered care
A
- provide patients with nutritious food that is familiar and congruent with their beliefs
- include patient in the decision making process when dietary modifications are required
- change in ability to self-feed and/or the need to consume an altered diet may influence a patients perception of quality of life

Safety Guidelines
A
- assess for s/s of malnutrition and identify patients at risk
- identify patients at risk for dysphagia and collaborate w/ other members of the health care team to minimize complications
- ensure the correct patient and the correct diet
- assess the level of consciousness before feeding
