Check skin turgor, delayed cap refill, dry lips/and mucus membranes.
Labs for dehydration
CBC/BMP, BUN, Creatine, Urine Specific Gravity, Electrolytes
Interventions: dehydration
Increase fluid intake (Ask do you prefer ice water?-make them want to drink) Monitor I&O and educate patient.
Assess: Hypotension
Vitals, check for inadequate perfusion(altered LOC, cool/pale skin, decreased urine output) Observe for dizziness, syncope, orthostatic hypotension.
Interventions: Hypotension
Elevate legs, administer fluids/electrolytes, monitor vitals

With malnutrition it’s important to..
Involve the patient and order ensure or something the patient will like to eat.
Additional findings with malnutrition..
