  1. First level assessment ( S&S)
  2. Second level assessment ( causes)
  3. Nursing Diagnosis ( in nursing term)
  4. Goal setting: nursing aid the patient: long term outcome and short term outcome.
  5. Nursing intervention: The way we intervention, how we do that?
  6. Evaluation: The intervention we did, it worked? It base on the intervention we did.

Characteristics of Nursing Process?

A

CIRCULAR NOT LINEAR
- ■ continuous

■ previous assessment is constantly re-examined for accuracy

Characteristics of Nursing Process
client status oriented:

■ look at health needs of the client

■ client’s needs always take precedence

■ care plans are organized according to statements about the client’s health status

Characteristics of Nursing Process
A cognitive process:

■ involves the use of intellectual skills in problem-solving and decision making

■ nurse uses critical thinking to apply nursing knowledge, develop, and implement the plan of care
Critical Thinking
■ the ability to think in a systematic and logical manner
■ Being open to ideas and engaging in questioning
■ Reflect on reasoning process
■ Involves the integration of relevant knowledge (biology, psychology, sociology etc), recall of prior clinical experience, creativity, flexibility, perseverance,
■ Allows the nurse to see the “big picture” of a patient’s clinical situation
■ Encourages the nurse to connect pieces of data to form a conclusion
