A

Contraindications:
• Allergy to any NSAID or salicylate, and celecoxib is also contraindicated in the presence of allergy to sulfonamides
• CV dysfunction or hypertension, peptic ulcer, GI bleeding
• Pregnancy, lactation
Cautions:
• renal and hepatic dysfunction

DDI:
Decreased diuretic effect when these drugs are taken with loop diuretics; there is a potential for decreased antihypertensive effect of beta-blockers if these drugs are combined; and there have been reports of lithium toxicity, especially when combined with ibuprofen

A

Assess:
Hx: allergies to any salicylates, NSAIs or tartrazine, pregnancy and lactation, hepatic or renal disease; CV dysfunction, hypertension; GI bleeding or peptic ulcer
Physical: vital signs, skin, temp, orientation and reflexes, perfusion, respirations and adventitious sounds; liver evaluation, BS (baseline)
Labs: kidney, liver function

A

Implementation:
• Administer with food if G.I. upset is severe; provide small, frequent meals
• Check all drugs being taken for possible salicylate ingredients
• Monitor for severe reaction
• Ensure the patient is well hydrated during therapy
• Provide thorough patient teaching
