LO2: curr struc and funcs of NHS in england.

-curr reforms:
Commissioning- key part of markets. Commissioners act as customers choosing btw care providers on pt behalf. On basis of pt need, cost and qual.
Health and soc care act 2012- devolves pow (esp commiss) to GPs and others in prim care. Alts NHS struc. Incr use of markets with opps for staff to set up own organisats (soc enterp). Reqs effic savings up to 20bn per yr on budget of 100bn.
- NHS struc:
Sec of state for health: overall accountabil for NHS.
DOH: sets nat stands, shapes NHS and soc care, sets nat tariff (fee for services eg hosp trusts, CCGs).
NHS england: authorises CCGs. Supps, devs and perf manages commissioning. Commissions spec services, prim care etc.
CCGs: commiss secondary and comm HC services. Must acc for nat guidance in their decis. Responsib for 65% flow NHS budget. Public health now responsib of oc authorities. GPs commiss by NHS england.
Providers organisations: no change but new opps for competit.
-service provis-
CCGs and NHS eng commiss pay NHS trusts, community HC trsust and other providers for care of their popn. Commissioners can place contracts with private/vol sector providers too.
NHS providers most money from CCGs and NHSE. Also from training. Or by gaining foundation trust status.

LO3: egs of managem activities and funcs of doctors in the NHS.

A

-curr devs- opps to be a partner or leader. Roles in resource alloc and contract managem esp in prim care.
- managem roles of doctors- medical/clinical director. Consultant. GP principal or partner. Any lev has some managerial responsibs. Many other senior and middle managerial roles rarely filled by Drs eg chief exec, gen managers (though often by nurses and AHPs).
-clinical directors- hosp trusts div to clincal directorates based on specialities. Dr director leads. Also have lead nurse and gen manager. Clinic directors prov training, design and implem policy, clinic audit, dev guidelines, new dr induction.
-medical director- qual of care. Comm btw board and staff. Leadership and implem change. Work with HR. Oversees hiring, discip, policy, clinic stand.
-managem skills- strategic, financial, operational, human resources.
Team managem- comm, clear chain of responsib and leadership, systmes for collab, systems for monit/rev/impr, supp, dev, clear objectives.

-1948 as part welfare state.
- princips: universal, comprehensive, free at pt of deliv.
-secretary of state for health duty to prov health services.
Init run centrally by DOH.
Some changes arnd what constits a health need in 50s/60s.
-incr role for managers- more strategic managem roles.
-incr marketisat of provision- comp btw hosps aimed at impr choice and qual, containing costs. Sep of commissioners and providers of HC.
