How to distinguish median nerve problem at the carpal tunnel vs the forearm?
Flexor of the thumb is innervated before the carpal tunnel, so if weakness on flexion, then lesion is proximal.

First step of management of cauda equina and conus syndromes?
IV steroids then surgical decompression. Both emergencies.
Hyper/hypokalemic periodic paralysis
Caused by a mutation in a sodium/calcium channel. Weakness and paralysis precipitated by rest after exercise and high carbohydrate meals.

Basically like guillain barre but starts in arms and moves up to involve proximal muscles. Idiopathic brachial plexopathy

Contralateral pons or internal capsule.
CSF for MS?
Slight increase in protein, slight increase in WBC, normal glucose.
REM sleep behavior disorder
Shouting, flailing, etc. during sleep. Characteristic of synucleinopathies.
Effect of estrogen birth control on migrane?
Will make it worse. Stop estrogen if aura develops or if migraine worsens.
