Neuropathies Flashcards
isolated anterolateral thigh numbness without numbness
Meralgia Paresthetica ( a compressive neuropathy of the lateral femoral cutaneous nerve)
Treat by relieving pressure
Sensory loss over palmar surface of first three digits and weakness with thumb abduction and opposition
Median Neuropathy ( carpal tunnel syndrome)
Wrist splints or glucocorticoid injections for mild disease
surgical release if severe
Numbness of 4th/ 5th fingers, interosseous muscle weakness
Ulnar Neuropathy - Elbow Splinting or Elbow pads
Surgical Release if severe
pain tingling/ numbness over great toe of foot and along medial foot
tarsal tunnel syndrome
Local glucocorticoid injection
upper and lower face weakness
Bells Palsy
Prednisone if within 72 hours
Multiple noncontiguous nerve deficits (mononeuritis multiplex)
Consider Vasculitis (especially if painful), lymphoma, amyloidosis, sarcoidosis, lyme disease, HIV, leprosy and diabetes
Distal and Symmetric (stocking glove) sensory or sensorimotor
Axonal Polyneuropathies ; DM and Alcohol are the most common causes, small fiber neuropathy will present with pain only
Severe Unilateral Leg pain, numbness, proximal weakness, atrophy and weight loss
Diabetic Lumbosacral Radioplexus Neuropathy
Acute Ascending, Areflexic paralysis and paresthesias often preceded by GI illness (Campylobacter infection) CSF shows elevated protein and a normal cell count (albuminocytologic dissociation
Guillain Barre Syndrome
Treat with Plasma Exchange or IVIG
Symmetric Distal Sensory Neuropathy in the setting of MGUS, Multiple Myeloma, amyloidosis and cryoglobulinemia
Paraproteinemic Neuropathy
Treat underlying disorder `
