NPH
The early gait changes area similar to parkinsons, short shuffling steps, wide base, retropulsion, abnormal pull test. If has ANY urinary symptoms make sure to suspect NPH and get an MRI
Management of dissection
Treat with antiplatelet agents. Endovascular repair is reserved for patient who have recurrent ischemia despite treatment.
Anti-NMDA receptor encephalitis
Limbic encephalitis p/w subacute memory distubance, personality change, psychosis, encephalopathy and seizures. Oral dyskinesia is common. Paraneoplastic, associated with ovarian teratomas. Remove both ovaries if teratoma and plasma pharesisis.
Anti-Hu paraneoplastic encephalitis
Usually motor or sensory (although can be limbic). Associated with small cell lung cancer.

Watchful waiting if anterior circulation and

Amantadine and modafinil
Secondary stroke prevention
Clopidogrel or ASA+dipyridamole rather than just ASA alone
Posterior column demylentination
B12 def, syphilis, and copper deficiency
Anti-epileptic choice
Lamotrigine is best tolerated in elderly
Carbamazapine is best in women of reproductive age
