Most common cause of bacterial meningitis in:
- Neonates
- Children >1 yr and adults
- Elderly
- Chronic lung disease
- Sinusitis, otitis media
- Neurosurgical patients
- Immunosuppressed / elderly
- Group B strep
- Strep pneumoniae, Nesseria meningitidis
- S. pneumoniae and gram-(-) enteric bacilli
- Pseudomonas
- Nontypeable H. influenzae, S. pneumoniae
- S. pneumoniae, nontypeable H. influenzae, S. aureus
- Listeria monocytogenes
Unlike other bacterial meningitidis, meningitis caused by this organism presents with lymphocytic pleocytosis and often normal CSF glucose.
Listeria monocytogenes
Total duration of therapy in bacterial meningitis
10 - 14 days
Empiric regimen in bac meningitis
Vanco 15mg/kg +
Cefotaxime 50mg/kg / Ceftriaxone 100mg/kg
Test of choice for brain abscess
MRI with contrast
(+) Ring-enhancing lesion
Empiric regimen for brain abscess. Duration of therapy?
Metronidazole + Third-Gen cephalosporin
4 - 8 weeks
Most common neurological syndrome in HIV infection
Encephalopathy and AIDS dementia complex
The presence of nonclearing (atraumatic) RBCs in CSF suggest infection with this organism.
HSV
Causes hemorrhagic encephalitis
Diagnostic test of choice for HSV encephalitis
HSV PCR of CSF
Schedule of rabies vaccine administration, post-exposure prophylaxis
As soon as possible, day 3, 7, 14 28
Human rabies immunoglobulin also given at the same time as the first dose; or within 7 days.
Most common neurologic manifestation of Syphilis
Meningitis
