Fully awake, responds to all stimuli, able to follow verbal commands.
Alert
Drowsy or asleep most of the time, can be awaken by gentle shaking or saying their name
Lethargic
Extreme drowsiness, minimally responsive, barely follows commands, vigorous stimulation to awaken, difficulty staying awake
Obtunded
Unconscious most of the, minimally responsive, barely follows time, no spontaneous
movement, awakens briefly only with repeated vigorous stimulation, responds in groans, moans
Semi-comatose
No response to verbal or painful stimuli, cannot be awakened, does not speak, some reflexes may be present, abnormal posturing may be present
Unconscious
Your patient has damage to the upper brain stem. Their arms are addicted and extended. Wrists are pronated with fingers flexed. Legs are stiffly extended with plantar flexion of feet
Decerebrate
You patient has damage to one or both corticospinal tracts. There arms are addicted with elbows flexed.
Decorticate
