Nursing care:
Education
Check with HCP has ordered to stop sedatives, sleeping aids and tranquilizers.
Avoid caffeine for 8 hours
No oils, creams, conditioner in hair for electrode placement

Know patient (medications, what to hold, keep PT safe)

Eye opening= Spontaneously-4; Speech-3; To pain-2, No eye opening-1
Verbal= Oriented ×3-5; Confused-4; Inappropriate words-3 Incomprehensible sounds-2, No response-1
Motor response= Obeys commands-6; Moves to localized pain-5; Flexion withdraw from pain-4: Abnormal flexion (decorticate)-3; Abnormal extension (decerbrate)-2; No response-1
Defines LOC giving numeric scale.

Possible score 3-15 15 Normal 3- Totally unresponsive <7-9 Comatose Every brain 🧠 is different but generally classified as Severe GCS 8 or less Moderate 9-12 Mild GCS 13-15
