Dose of amitriptyline
Depression: 75-200mg in divided or pre-bedtime doses
Neuropathic pain: 10mg at night
Migraine prophylaxis: 10mg at night (may be increased)
Dose of citalopram
PO 20mg OD up to 40mg OD
Dose of zopiclone
3.75-7.5mg nocte
Mechanism of action of quetiapine
Short acting atypical antipsychotic.
Dopamine, serotonin and adrenergic antagonist, potent antihistamine
Dose of quetiapine
Schizophrenia: Max 750mg in divided doses. Usual range 300-450mg
Mania: max 800mg in divided doses. Usual range 400-800mg
BP Depression: max 600mg in divided doses. Usual dose 300mg
Mechanism of action of olanzapine
Atypical antipsychotic. High affinity for serotonin and dopamine (less for latter). Potent antagonist for M3
Dose of olanzapine
PO: 5-20mg
Rapid control IM: 10mg, 5mg in elderly. Repeat after 2hrs if necessary
Deep IM: 210-300mg every 2 weeks then maintenance dose lower but adjusted to response
Dose of venlafaxine
75mg/day. Max 375mg/day
Dose of mirtazapine
15-30mg nocte up to 45mg
Dose of Haloperidol
PO: maintenance 1-3mg/day, max 20mg
Deep IM: 50mg/4wk up to 300mg/4wk
Dose of chlorpromazine
PO: max 300mg/day, start 25mg TDS
Deep IM: 25-50mg/6-8h
Dose of gabapentin
Initially 300mg OD, usual dose 0.9-3.6g/day in 3 doses. Max 4.8/day.
Dose of sodium valproate
PO: 1-2g/day, max 2.5g
IV: 10mg/kg, max 2.5g
Dose of phenytoin
Initial dose: 20mg/kg (max 2g) slow IV
Maintenance: 100mg PO/IV /6-8h
