what is the first line treatment for Bell’s Palsy
Prednisone
Sharp headache and visual changes after recent sinsitis or facial infection.
lateral gaze palsy may or may not be present
Cavernous sinus thrombosis
PE: fever, opthalmoplegia, ptosis, chemosis, and periorbital edema
Younger adult presents with anterior mediastinal mases causing local compressive symptoms (dysphagia or dyspnea)
PE: enlarged painless supravlavicular lymphadenopathy
Hodgkin’s lymphoma
Classic B symptoms of hodgkin’s lymphoma: fever, drenching night sweats, and weight loss
myasthenia gravis (descending weakness) or progressive exertional weakness that improves with rest
dysphagia/dyspnea
no night sweats, fever, and weight loss
Thymoma
A solitary ring-enhancing lesion in the brain wiht an associated mass effect and focal neurologic deficits in a patient with HIV/AIDS
Primary CNS lymphoma
MRI will show ring -enhancing lesions
definitive diagnosis is made using stereotactic brain biopsy with tissue analysis
What is the treatment for acute angle closure glaucoma
treatment is aimed at opening the anterior chamber angle and lowering the intraocular pressure.
Optimum treatment includes systemic acetazolemide, and optic timolol and pilocarpine to induce miosis and open the anterior chamber angle.
What is the drug of choice for patients with agitation due to acute delirium?
Haloperidol
Pain/cramping in the legs that is exacerbated by walking and leaning forward or sitting relieves the symptoms
Lumbar spinal stenosis
Diagnosis is confirmed with MRI
what lobe of the brain contains broca’s area?
the frontal lobe.
Broca’s aphasia presents with non-fluent, broken speech, and poor repetition. Comprehension is preserved.
what are the signs and symptoms of an anterior cord lesion?
Anterior cord lesions presents with a loss of motor function, and loss of pain and temperature bilaterally below the lesion.
This type of spinal cord injury can be seen in patients who are having a thoracoabdominal aneurysm repair.
What are the signs and symptoms of a posterior cord lesion?
loss of proprioception and vibration sense only (these are carried in the DCMS) and its very rare… so don’t pick it on the exam like i did.
