Nerve Palsies in the Limbs Flashcards
RECAP- five terminal branches of the brachial plexus?
Musculocutaneous
Axillary
Radial
Median
Ulnar
Which two parts of life are brachial plexus lesions most common?
Childbirth- being pulled out through a tight space
Falling onto shoulder (…this isn’t a part of life tho)
Which nerve roots may be damaged during childbirth?
C5-6
->supplying shoulder muscles, deltoid, brachialis and biceps)
What is the name of one of the most common neurological birth injuries affecting the brachial plexus?
Erb palsy/ Erb-Duchenne paralysis
What happens to the arm in Erb-Duchenne paralysis?
Arm is adduced as shoulder no longer works
Elbow is extended as biceps isn’t working
Forearm pronated
Wrist flexed
Which nerve palsy can occur due to upward traction e.g. birthing in breech position, pulling on arm?
Klumpke’s palsy
->pulling of arm is like if trying to hold onto a branch to prevent falling
What is the sign of Klumpke’s palsy?
Claw hand
Axillary nerve roots?
C5,6
Where does the axillary nerve wrap around?
Surgical neck of humerus
Which muscles does the axillary nerve supply?
Deltoid
Teres minor
Skin over lateral area- regimental badge area
In which injury in the axillary nerve often damaged?
Shoulder dislocation
What happens to the deltoid muscle in axillary nerve injury?
Deltoid atrophy
Radial nerve roots?
C5-T1
Which cord(s) does the radial nerve come from?
Posterior cord
Which arm muscle does the radial nerve supply?
Triceps
Which part of the humerus does the radial nerve run along?
Radial groove
Which muscles in the forearm does the radial nerve supply?
Brachioradialis
Extensor carpi radialis longus
Extensor carpi radialis brevis
When may there be radial nerve palsies?
Humerus fracture- can become entrapped
The radial nerve can be compressed and cause which palsies?
Saturday night palsy
Honeymoon palsy
Crutch palsy
->commonly individual falls asleep on a Saturday night after having a few too many drinks with arm over back of chair, providing pressure on the radial nerve for a prolonged period
The symptoms of a radial nerve palsy depend on the site of the compression.
What would be the symptoms if the radial nerve was compressed in the axilla?
Loss of elbow extension
Loss of wrist extension
Sensory changes on forearm and hand
->this is because loss of triceps
The symptoms of a radial nerve palsy depend on the site of the compression.
What would be the symptoms if the radial nerve was compressed in the arm?
Loss of wrist extension
Sensory loss
The symptoms of a radial nerve palsy depend on the site of the compression.
What would be the symptoms if the radial nerve was compressed in the forearm?
Loss of finger extension
The symptoms of a radial nerve palsy depend on the site of the compression.
What would be the symptoms if the radial nerve was compressed in the wrist?
Loss of sensation
->can occur if wearing handcuffs
Which area of the hand is tested for fine touch and sensation of the radial nerve?
First web space on the dorsal aspect
Which cord(s) does the median nerve come from?
Medial and lateral cords
What dos the medial nerve supply in the forearm?
Flexors
LOAF muscles (lateral two lumbricals, opponens pollicus, abductor pollicus brevis, flexor pollicus brevis)
->except for flexor carpi ulnaris and medial half of flexor digitorum profundus)
Which digits does the median nerve supply sensation to?
Radial 3.5 digits
