Median nerve low lesion
n all thenar muscles palsy except ADD Pol.
Median Nerve
Origin- Brachial plexus C5-T1 roots-medial and lateral cords
Radial nerve low lesion
wrist extensors are not affected, only fingers and thumb extension at MCPJ level are affected- wrist
drop
radial nerve high lesion
triceps, Wrist extensors( ECRL/B), extensors of the fingers at MCPJ level and thumb extension/abduction are
not working= wrist and finger drop
wasting of hypothenar muscles
Flattening of Metacarpal arch, Abduction of little finger
wasting of intrinsic muscles
Clawing of the fingers
wasting of adductor pollicis
Ulnar nerve pathology
Fromet’s sign
Failure of adductor pollicis –> ulnar nerve pathology
Jeanne’s Sign
Thumb MP hyperextension with key pinch or gross grip
Wartenberg’s Sign
Inability to adduct the extended little finger
Pollock’s Sign
Inability to flex the DIP of the ring and small fingers in high palsies
