how does glomerulonephritis differ from nephrotic syndrome?
glomerulo= lose protein
nephro= losing an ass load of protein (has bigger holes for protein to come out of) more than glomerulo
explain the patho of nephrotic syndrome.
inflammatory response in glomerulus= cause big holes to form= an ass load of protein start leaking from the big holes= causes your body to lose A LOT of protein and you become hypo-album (and we know protein is important for fluid maitnenace in the vascular space)= low protein causes fluid shift to the tissues and you get anasarca.
Since all of the fluid is now in the tissues, what has happened to the circulating blood volume?
decreased
since we now have low circulating blood volume, what do the kidneys do?
they secrete alodosterone causing you to retain sodium and water.
why do we want this patient on bedrest?
bed rest induces diuresis
why would this patient go on prednisone?
-decrease inflammation and it also shrinks the holes so protein cannot get out
what is the main complication of prednisone or other steroids?
immunosupression
what are we gonna do with the protein in the diet?
increase it
the common rule with protein and kidney disease is__
to limit protein with kidney problems except with nephrotic syndrome
A lasix and albumin infusion… how would this help?
albumin= pull fluid back into the vascular space
lasix= without it, they’d get hypervolemic
