Neonatal hypoglycaemia

A

Common to have transient hypoglycaemia

Presentation:

  • Irritability
  • Pallor
  • Poor feeding
  • Drowiness
  • Hypotonia
  • or asymptomatic

Investigations:

  • BMs
  • Consider maternal drug use as it’s like withdrawal

Management:

  • If asymtomatic - encourage normal feeding, montor BMs
  • If symptomatic - admit to NNU, give IV infusion of 10% dextrose

Hypoxic ischaemic encephalopathy

A

Irreversible brain damage from antenatal or perinatal hypoxia

Causes:

  • Placental abruption
  • Cord compression
  • Prolonged respiratory arrest

Presentation:

  • encephalopathy - seizures, abnormal neuro exam
  • Multi -organ dysfunction - resp failure, renal, GI ischamea, metabolic, haem

Investigations:

  • EEC
  • Brain MRI
  • Other investigations depending on presentation
  • Supportive care
  • Maintain breathing and circulation
  • Cool to 33-34 deg within 6 hrs of birth to prevent further damage by secondary reperfusion injury
