Neonatal hypoglycaemia
A
Common to have transient hypoglycaemia
Presentation:
- Irritability
- Pallor
- Poor feeding
- Drowiness
- Hypotonia
- or asymptomatic
Investigations:
- BMs
- Consider maternal drug use as it’s like withdrawal
Management:
- If asymtomatic - encourage normal feeding, montor BMs
- If symptomatic - admit to NNU, give IV infusion of 10% dextrose

Hypoxic ischaemic encephalopathy
A
Irreversible brain damage from antenatal or perinatal hypoxia
Causes:
- Placental abruption
- Cord compression
- Prolonged respiratory arrest
Presentation:
- encephalopathy - seizures, abnormal neuro exam
- Multi -organ dysfunction - resp failure, renal, GI ischamea, metabolic, haem
Investigations:
- EEC
- Brain MRI
- Other investigations depending on presentation
- Supportive care
- Maintain breathing and circulation
- Cool to 33-34 deg within 6 hrs of birth to prevent further damage by secondary reperfusion injury
