How do you investigate neonatal jaundice?

A

<24 hours OR <35 weeks of life- serum bilirubin
>24 hours- transcutaneous bilirubin

if TCB => 250 umol-> serum

Check underlying causes:
1-HCT
2- blood groups
3- DAT test 
4- Blood film for Hereditary spherocytosis 
5-MC&S- ? sepsis

What is the management of neonatal jaundice?

A

1- Reassurance

Unconjugated= check with therapy lines

Photo or exchange transfusion

Conjuagted=? Biliary atresia

How does phototherapy work? + what’s the follow-up?

A

light waves of blue- green band coverts unconjugated bilirubin into water-soluble forms.

Key to protect babies eyes and measure bili 4-6 hours after starting
