Paeds BLS procedure
1- SAFE
2-Response
3- Open airway + check breathing
4-5 rescue breaths
5- Asses life sign (<1= carotid + femoral) and (>1= brachial + femoral)
6- Chest compressions 15:2 , if alone its 30:2
What are the signs of shock?
Altered consciousness ]sunken eyes
dry mucus membranes
Reduced skin turgour
What is the detailed management of shock?
1- CALL SENIOR HELP
2-IV saline Bolus < 10 minutes of 20ml/Kg
3- Maintenance fluids (24 hours) - 100ml/Kg - 10 kg , 50ml/Kg for +10 Kg and 20 ml/Kg for each kilo on top
4-ICU if failing, give inotropic support
What is anaphylaxis?
Anaphylaxis is a life threatening systemic hypersensitivity reaction that causes airway, breathing and circulation problems.
What is the management of anaphylaxis?
ABCDE approach
1- High flow O2
2-IM Adrenaline 1:1000 (anterolateral aspect of the thigh)
REPEAT IM injection in 5 minute intervals till response
3- IV fluids
POST Resus:
1- Slow IM/IV hydrocortisone
2- Neb salbutamol if wheezy
What is the management of status epilepticus?
0 mins- high flow O2
5 mins- Buccal midazolam/ Diazepam rectal
15 mins- IV Lorazepam-> IO + CALL SENIOR HELP
25 mins- IV Phenytoin + Senior help/ anaesthetist
45 mins - rapid sequence induction by anaesthetist with thiopentone
