What is necrotising fasciitis
Life threatening SC soft tissue infection - may extend to deep fascia but not muscle
Rapidly progressive
Presentation of nec fasc
Hot, red, swollen
Exquisitely tender, often outside margin of erythema, rapid progression w skin discolouration
Often septic and unstable
Diagnosis of nec fasc
Clinical
Imaging may be heplful - air bubbbles under skin if anaerobes - but ust not delay treatment
Pathogens causing nec fasc
Group A strep
Enterbacteriaceae (gram negative bowel)
Anaeribes
S.aureus
Often multipathogenic - multiple organisms, broad antibiotics w antittoxin coverage required
Treatment for nec fasc
Surgical debridement
Culture - furture antibitoics
Antibiotics for nec fasc
Tazocin/meroponem + clindamycin or linezolid
