A

Why do we record our decisions?

A

To maintain accountability / scrutiny / confidence / objectivity – and to avoid bias.

For transparency – so a third person (or the officer themselves at a later date) can appreciate why a decision was made.

Gives structure and accounts for legislation used, and means that the Code of Ethics is intrinsic to the process.
