Nasopharynx Flashcards
Nasopharynx staging: Tumor confined to the nasopharynx or extension to oropharynx and/or nasal cavity without parapharyngeal involvement
T1
Nasopharynx staging: Tumor with extension to parapharyngeal space, and/or adjacent soft tissue involvement (medial pterygoid, lateral pterygoid, prevertebral muscles)
T2
Nasopharynx staging: Tumor with infiltration of bony structures at skull base, cervical vertebra, pterygoid structures, and/or paranasal sinuses
T3
Nasopharynx staging: Tumor with intracranial extension, involvement of cranial nerves, hypopharynx, orbit, parotid gland, and/or extensive soft tissue infiltration beyond the lateral surface of the lateral pterygoid muscle
T4
Nasopharynx staging: Metastasis in unilateral cervical lymph nodes and/or unilateral or bilateral metastasis in retropharyngeal lymph nodes, 6 cm or smaller in greatest dimension, above the caudal border of cricoid cartilage
N1
Nasopharynx staging: Metastasis in bilateral cervical lymph nodes, 6 cm or smaller in greatest dimension, above the caudal border of cricoid cartilage
N2
Nasopharynx staging: Metastasis unilateral or bilateral metastasis in cervical lymph nodes, larger than 6 cm in greatest dimension, and/or extension below the caudal border of cricoid cartilage
N3
Nasopharynx staging: T1N0M0
Stage I
Nasopharynx staging: T1N1 or T2N0-1
Stage II
Nasopharynx staging: T1-2N2 or T3N0-2

Nasopharynx Tx paradigm: T1N0M0
Definitive RT (elective node, bilateral RP and II-IV)
Nasopharynx Tx paradigm: T2N0M0
Definitive RT +/- systemic therapy (bulky tumor, high serum EBV DNA copy number)
Nasopharynx Tx paradigm: T1-2N1 or T3N0
CRT, consider induction or adjuvant CT (bulky/high serum EBV)
Nasopharynx Tx paradigm: T3N1-3, T4N0-3, T1-2N2-3
Clinical trial or Induction CT followed by CRT (preferred)
Nasopharynx Tx paradigm: Induction chemo agents
Gem/Cisplatin preferred
Nasopharynx Tx paradigm: Concurrent and adjuvant agent
Cisplatin, then Cis/5FU
