what is nappy rash
also known as napkin dermatitis
-acute inflammatory reaction of the skin in the nappy area
-form of irritant contact dermatitis
what causes nappy rash
-weakened skin barrier due to skin maceration (when the skin becomes soft due to prolonged exposure to moisture)
-prolonged contact with urine and faeces
-friction between skin and nappy
how long does nappy rash occur
2-4 days
-candida infection (yeast) = 10-21 days
risk factors of nappy rash
-infrequent nappy changes
-nappy type
-skincare practices
-no nappy free time
-dermatological conditions e.g. eczema
diagnosis/symptoms for general nappy rash
-red/brown areas
-skin erosion
-child is agitated
-dry itchy rash
management for nappy rash
-leave nappy off for some time
-change nappy frequently
-avoid using soap to clean nappy area, only cotton wool and water
-maybe change nappy type
-use barrier cream or ointment before bed e.g. Sudocream
treatment for nappy rash
-barrier ointments e.g. dimeticone and white soft paraffin ointments
treatment severe nappy rash with extreme discomfort
-for severe nappy rash consider hydrocortisone 1% cream and barrier preparation (to be applied after the hydrocortisone)
fungal nappy rash treatment
Use topical antifungal imidazole (used to treat skin infections) e.g. clotrimazole, econazole, miconazole for 7-10 days tds/bd . Advise against the use of barrier protection until the infection has settled.
treatment for bacterial nappy rash
oral antibiotics e.g. flucloxacillin 7/7 and clarithromycin
symptoms for fungal nappy rash
-bright red patches around nappy area
-papules and pustules spread into skin folds
