How are Discrepancy Reports categorized?
Category 1 (CAT 1 or CAT I) & Category 2 (CAT 2 or CAT II)
When is a CAT I Discrepancy Report used and when must it be submitted?
When discrepancies create a safety risk that could result in death or injury to personnel, risk of damage to or loss of aircraft, equipment, or facilities. Must be submitted within 24 hours of discovery.
When is a CAT II Discrepancy Report used and when must it be submitted?
When discrepancies do not create a safety risk or potential for damage, but do pose a widespread risk to the quality of maintenance and operational readiness. Submit within 5 calendar days of deficiency discovery.
What Discrepancy Reports are reported and monitored via the Joint Deficiency Reporting System (JDRS)?
- HMR
- PQDR (CAT I and II only)
- TPDR
- AIDR
- ADDR
- BTR
When Joint Deficiency Reporting System is not accessible how are CAT I & II DR’s submitted?
When JDRS is not accessible, CAT I and CAT II DRs will be transmitted by Naval Message via HMR Report Message Template.
What information is the Report Control Number (RCN) comprised of?
- Service Designator Code (N, V, or R) of originating activity
- DODAAC/UIC of originating activity
- Calendar Year
- Locally assigned control numbers (numeric only)
What do the Service Designators N, V, and R signify?
- “N” - Navy/Marine Corps Non-Deploying Units
- “V” – Navy/Marine Corps Atlantic Fleet Operating Forces
- “R” – Navy/Marine Corps Pacific Fleet Operating Forces
How many characters does an RCN contain?
12 for regular reports and 14 for Supplemental AIDR Reports
When are Hazardous Material Reports used?
HMR’s are submitted on material deficiencies, which if not corrected, could result in death or injury to personnel, or damage to or loss of aircraft, equipment, or facilities.
How are Hazardous Material Reports Categorized?
All HMR are considered CAT I safety discrepancies and must be submitted within 24 hours of discovery.
What conditions would require a Hazardous Material Report?
- Malfunction or failure of a component, which, if not corrected, could result in death or injury to personnel, or damage to or loss of aircraft, equipment, or facilities.
- A configuration deficiency in aeronautical equipment (aircraft, engines, support equipment (SE), or components) that creates a safety hazard.
- A design flaw that allows incorrect installation of parts resulting in possible system malfunction or failure.
- In-flight or ground loss of aircraft parts (things falling off aircraft) in which maintenance or material factors are involved.
When are Product Quality Deficiency Reports Used?
PQDR’s are submitted for deficiencies in new or newly reworked material, products, and software.
When are Technical Publications Deficiency Reports Used?
TPDR’s are submitted to correct deficiencies in technical publications.
How many Categories of Technical Publications Deficiency Reports is there?
CAT 1 (Safety Related), CAT 2, CAT 3, & CAT 4 (Non-Safety Related)
