How are Discrepancy Reports categorized?
Category 1 (CAT 1 or CAT I) & Category 2 (CAT 2 or CAT II)
When is a CAT I Discrepancy Report used and when must it be submitted?
When discrepancies create a safety risk that could result in death or injury to personnel, risk of damage to or loss of aircraft, equipment, or facilities. Must be submitted within 24 hours of discovery.
When is a CAT II Discrepancy Report used and when must it be submitted?
When discrepancies do not create a safety risk or potential for damage, but do pose a widespread risk to the quality of maintenance and operational readiness. Submit within 5 calendar days of deficiency discovery.
What Discrepancy Reports are reported and monitored via the Joint Deficiency Reporting System (JDRS)?
- HMR
- PQDR (CAT I and II only)
- TPDR
- AIDR
- ADDR
- BTR
When Joint Deficiency Reporting System is not accessible how are CAT I & II DR’s submitted?
When JDRS is not accessible, CAT I and CAT II DRs will be transmitted by Naval Message via HMR Report Message Template.
What information is the Report Control Number (RCN) comprised of?
- Service Designator Code (N, V, or R) of originating activity
- DODAAC/UIC of originating activity
- Calendar Year
- Locally assigned control numbers (numeric only)
What do the Service Designators N, V, and R signify?
- “N” - Navy/Marine Corps Non-Deploying Units
- “V” – Navy/Marine Corps Atlantic Fleet Operating Forces
- “R” – Navy/Marine Corps Pacific Fleet Operating Forces
How many characters does an RCN contain?
12 for regular reports and 14 for Supplemental AIDR Reports
When are Hazardous Material Reports used?
HMR’s are submitted on material deficiencies, which if not corrected, could result in death or injury to personnel, or damage to or loss of aircraft, equipment, or facilities.
How are Hazardous Material Reports Categorized?
All HMR are considered CAT I safety discrepancies and must be submitted within 24 hours of discovery.
What conditions would require a Hazardous Material Report?
- Malfunction or failure of a component, which, if not corrected, could result in death or injury to personnel, or damage to or loss of aircraft, equipment, or facilities.
- A configuration deficiency in aeronautical equipment (aircraft, engines, support equipment (SE), or components) that creates a safety hazard.
- A design flaw that allows incorrect installation of parts resulting in possible system malfunction or failure.
- In-flight or ground loss of aircraft parts (things falling off aircraft) in which maintenance or material factors are involved.
When are Product Quality Deficiency Reports Used?
PQDR’s are submitted for deficiencies in new or newly reworked material, products, and software.
When are Technical Publications Deficiency Reports Used?
TPDR’s are submitted to correct deficiencies in technical publications.
How many Categories of Technical Publications Deficiency Reports is there?
CAT 1 (Safety Related), CAT 2, CAT 3, & CAT 4 (Non-Safety Related)
When is a CAT 1 Technical Publications Deficiency Report Used?
CAT 1 TPDR’s are submitted for safety related technical publication deficiencies that could result in death or injury to personnel, or damage to or loss of aircraft, equipment, or facilities.
When is a CAT 2 Technical Publications Deficiency Report Used?
CAT 2 TPDR’s are submitted for non-safety related technical publication deficiencies that cause maintenance delays of 8 hours or more (Measurement Values & P/N discrepancies).
When is a CAT 3 Technical Publications Deficiency Report Used?
CAT 3 TPDR’s are submitted for non-safety related technical publication deficiencies that cause maintenance delays of less than 8 hours (P/N discrepancies).
When is a CAT 4 Technical Publications Deficiency Report Used?
CAT 4 TPDR’s are submitted for non-safety related technical publication deficiencies of a non-technical or administrative nature that do not delay maintenance (misspelled words or typographical errors).
What Discrepancy Reports are automatically considered for an Engineering Investigation (EI)?
All HMR’s and PQDR’s are automatically considered for Engineering Investigations.
For what other reasons may an Engineering Investigation be requested?
- For materials associated with aircraft or ground mishaps, lightning strikes, electromagnetic interference, and stray voltage problems.
- Components rejected by Navy Oil Analysis Program (NOAP) after all authorized repairs have been attempted.
- To support the mandatory investigation requirements of Activated Aircraft Emergency Egress Systems per OPNAVINST 3750.6.
Who is responsible for determining if an Engineering Investigation will be conducted, closed with a final response, or closed administratively?
The In-Service Support Center (ISSC) Engineer for Material or Equipment
How does the Work Center responsible for the component close out the maintenance Action for an Engineering Investigation?
- Mark the MAF with “EI” or “PQDR” in 3-inch Red Letters
- Annotate the Report Control Number (RCN) in the discrepancy block.
- Annotate BCM-8 7 EI or PQDR in Corrective Action block.
What is the Supply Departments role in an Engineering investigation process?
- Verify the exhibit is correctly packaged and marked in accordance with NAVSUP P-700.
- Quarantine from materials normally being processed.
- Hold the EI/PQDR Exhibit until shipping or disposition instructions are received from ISSC.
Who may the Maintenance Officer delegate JDRS DR Submitter privileges too?
AMO, QAO, and NAMDRP Program Manager.
What are the Responsibilities of the Quality Assurance Officer (QAO) regarding the Naval Aviation Maintenance Discrepancy Reporting Program (NAMDRP)?
- Designate the Quality Assurance Supervisor (QAS) or a Quality Assurance Representative (QAR) as the NAMDRP Program Manager. (12-month minimum billet)
- Review NAMDRP Computerized Self-Evaluation Checklist (CSEC) audit and direct corrective actions for process improvements.
What are the Responsibilities of the NAMDRP Program Manager?
- Perform a CSEC Program Manager assessment within 30 days of designation and Annually thereafter
- Assist Work Centers with preparing Discrepancy Reports
- Screen DRs and verify all data fields are complete and accurate
- Maintain an RCN Log
- Provide JDRS Training to Drafters and Submitters
- Maintain Program File
What minimum information must a Report Control Number Log contain?
- Date.
- RCN.
- DR Type.
- Nomenclature.
- BUNO Number (for AIDRs).
- P/N.
- S/N (if applicable).
- NSN or NIIN.
- Brief description of deficiency.
- Status (filled in upon issuance of final or closing report).
