Mechanism of action
Competitive opiate R antagonist > displaces opiate molecules, has greater affinity for R site, but shorter duration.

Opiate antagonist/ antidote
Pharmacokinetics
OOA- <2min IV, 2-10min IM/IN
Duration- 20-120min
H/L- 60-90min
Indications
- Partial/complete reversal of respiratory depression caused by opiates
- Coma of unknown origin
- R/O opiate involvemant in polypharmacy overdose prior to RSI
Contraindications
Hypersensitivity
Side effects
Hypotension, hypertension, ventricular dysrhythmias, N/V, seizures
Dose
Adult 0.4-2mg slow IV, IO q 2-3min or until respiratory status improves
2mg IM
2mg IN 1mg/nare
Pedi 0.1mg/kg IV, IM, IN max 2mg/dose
Precautions
Caution with pt potentially dependent on opiates, may induce withdrawal- including newborn infants with addicted mothers
