N/V Meds Flashcards
Anticholinergics/Antihistamines
scopolamine (Transderm-Scop)
diphenhydramine (Benadryl)
Anticholinergics/Antihistamines Action
act primarily on vomiting center
decreases stimulation of CTZ and vestibular pathways
Anticholinergics/Antihistamines SE
drowsiness
dry mouth
blurred vision
tachy
constipation
Anticholinergics/Antihistamines Nursing Interventions
anticholinergic nursing interventions!
Dopamine Antagonists
promethazine (Phenergan)
Dopamine Antagonists Action
blocks dopamine receptors in brain; inhibits signals to vomiting center in medulla (postoperative, chemo)
Dopamine Antagonists SE
dry mouth
sedation
constipation
OH
tachycardia
Dopamine Antagonists ADE
extrapyramidal symptoms with prolonged therapy (fall precautions!)
Serotonin Antagonists
zofran (Ondansetron)
Serotonin Antagonists Action
blocks serotonin receptors in CFZ and blocks afferent vagal nerve terminals in upper GI tract
Serotonin Antagonists SE
HA
dizziness
diarrhea
fatigue
Serotonin Antagonists ADE
allergic reaction
dysrhythmias
renal/liver damage
Serotonin Antagonists Nursing Interventions
QT prolongation if IV push too fast
CHEMO!
2-5 min IV push; lots of doses (extended)
Miscellaneous
metoclopramide (Reglan)
metoclopramide (Reglan) Action
suppresses impulses to CTZ; promotility agent
metoclopramide (Reglan) SE
fatigue
drowsiness
restlessness
metoclopramide (Reglan) ADE
neuroleptic malignant syndrome (NMS)
tardive dyskinesia
Emetics
Ipecac
Emetics Action
stimulates CTZ and acts directly on the vomiting center
induces vomiting after consumption of a toxic substance
Emetics Administration
use ipecac syrup, not fluid extract
take with a glass of water not with milk or a carbonated bev
vomiting occurs in 15-30 min; give activated charcoal if doesn’t work
gastric lavage may be needed if vomiting doesn’t occur
N/V Meds Overall Nursing Interventions
provide mouth care after vomiting
no alcohol
avoid motor vehicles/dangerous activities
monitor bowel sounds and vitals
FALL RISK!
