Myocardial & Pericardial Disease Flashcards
Peripartum or postpartum CMP treatment :
Bromocriptine
ECG findings of :
π« dilated CMP:
π« HOCM:
π« restrictive CMP:
π« dilated CMP: AFib
π« HOCM: LVH
π« restrictive CMP: low voltage QRS
Treatment of HCOM
- Γ-blockers
- non-DHP CCB βverapamilβ
- surgical myomectomy
- ICD if has syncope, VT/VF or FHx of sudden cardiac death
In patients of HCOM or restrictive CMP
Medications you should avoid : [β¦] & [β¦]
Digoxin
Nitrates
Criteria of Acute Pericarditis
2/4:
Pleuritic chest pain
Pleural effusion
Pericardial rub
ECG changes (diffuse ST elevation or PR depression)
[β¦] signs that present with both constrictive pericarditis and restrictive CMP
Kussmal
Paradoxical πJVD on inspiration
Diagnostic test with highest value for Pericarditis :
Cardiac MRI ( or CT )
15% of pericarditis present with [β¦]
Cardiac temponade
Pericarditis treatment :
NSAIDs / Colchicine
Steroid in some cases ( SLE / TB)
βNo NSAIDs for peri-infarction pericarditis
Triad of Cardiac temponade
Beckβs triad
1. Low BP
2. JVD
3. Muffled heart sounds
ECG findings of cardiac tamponade :
Electrical alternans
Best diagnostic test for Cardiac tamponade :
Echo
Compare Constrictive pericarditis & Cardiac tamponade presentation :
Constrictive Pericarditis β¦ Tamponade
Chronic Acute
No hypotension π»BP
π©Ί: normal Muffled
JVD JVD
JVP: prominent X & Y JVP: πΊX π«Y
Cardiac cath: elevation & equalization of ALL heart chambers
