What are the features of active myeloma
Hypercalcaemia Renal impairment Anaemia Bone pain and destruction Infections (CRABI)
What is the diagnostic criteria for multiple myelma
M protein in serum more than 30g/l
Bone marrow plasma cells more than 10%
Related organ damage (CRABI)
What does myeloma do to bone
Swtiches off new bone formation ie is lytic
What causes renal failure in multiple myeloma
Light chain depositin leading to renal tubules Dehydration Infection Hypercalcemia Uric acid Amyloid Acute tubular necrosis
How is multiple myeloma diagnoses i.e what are the tests
Blood test
Electropheresis : to seperate out protein according to size and charge that will quantify immunoglobulin and m-protein
Bone marrow biopsy
What translocation can occur in multiple myeloma
Between chromsome 14 and 23
What is renal failure treaated by
Avoiding nephortoxic drugs
Hydration
What is hypercalcemia treated with
Iv fluid replacement
Biphosphonate (reduced osteoclast acitivityin the bone to reduce bone lysis)
What is cord compression treated by
Mri scan
Radiotherapy
What is there hyperviscosity in multiple myeloma
Due to high levels of paraprotein
What is the treatment of hyperviscosity
Plasma exchnage
