For people with long-term conditions newly prescribed a medicine

What conditions are included in NMS? [4]
A
Asthma and COPD
T2 Diabetes
Antiplatelet/anticoagulant therapy
Hypertension

What four patient groups are MURs targeted at?
A
High risk medicine takers (NSAIDS, anticoagulants, antiplatelets and diuretics)
Recently discharged from hospital.
Respiratory disease
Risk of or diagnosed cardiovascular disease and prescribed at least four medicines.

What proportion of MURs must be to certain patient groups?
A
70%
