what is multiple myeloma?
haematological malignancy characterised by plasma cell proliferation
how does multiple myeloma arise?
due to genetic mutations which occur as b-lymphocytes differentiate into mature plasma cells
what is the median age at presentation?
70 years old
what mnemonic is used to remember multiple myeloma features?

what calcium abnormality is seen in multiple myeloma?
hypercalcaemia
* constipation
* nausea
* anorexia
* confusion
why is calcium abnormal in multiple myeloma?
increased osteoclastic bone resorption caused by local cytokines released by myeloma cells
what renal impairment is seen in multiple myeloma?
renal damage
* dehydration
* increasing thirst
what is the cause of renal damage in multiple myeloma?
- monoclonal production of immunoglobulins results in light chain deposition within the renal tubules
- amyloidosis
- nephrocalcinosis
- nephrolithiasis
what is the cause of anaemia in multiple myeloma?
bone marrow crowding suppresses erythropoiesis
what is the cause of bleeding in multiple myeloma?
bone marrow crowding results in thrombocytopenia
what bone features are seen in multiple myeloma?
- pain (especially in the back)
- increased risk of pathological fratures
what is the cause of bone pathology in multiple myeloma?
bone marrow infiltration by plasma cells and cytokine-mediated osteoclast overactivity creates lytic bone lesions
what is the cause of infection in multiple myeloma?
reduction in the production of normal immunoglobulins results in increased susceptibility to infection
what are the features of multiple myeloma?
- hypercalcaemia
- renal damage
- anaemia
- bleeding/bruise
- bone pain/fractures
- infection
- amyloidosis - e.g. macroglossia
- carpal tunnel syndrome
- neuropathy
- hyperviscosity
what findings represent multiple myeloma until proven otherwise?
raised ESR + osteoporosis
