Define MODS?
A clinical syndrome characterized by the development of progressive and potentially reversible physiologic dysfunction of 2 or more organs or organ systems that is induced by a variety of insults, including sepsis
What does MODS usually result from?
Infection
Injury
Hypoperfusion
Hypermetabolism
What is the aetiology of MODS?
The primary cause can trigger a systemic inflammatory response (sepsis or SIRS)
SIRS + infection –> severe sepsis –> MODS
What is the epidemiology of MODS?
Not very common
How are the presenting symptoms and Signs of MODS classified?
Multiple Organ Dysfunction Score
What is Stage 1 of the Multiple Organ Dysfunction Score?
Increased volume requirements, mild respiratory alkalosis, oliguria, hyperglycaemia, increased insulin requirements
What is Stage 2 of the Multiple Organ Dysfunction Score?
Tachypnoea, Hypocapnia, hypoxaemia, moderate liver dysfunction and haematologic abnormalities
What is Stage 3 of the Multiple Organ Dysfunction Score?
Shock, azotaemia, acid-base disturbance, significant coagulation abnormalities
What is azotaemia?
High nitrogen in the blood
What is Stage 4 of the Multiple Organ Dysfunction Score?
Vasopressor dependent
Oliguria
Anuria
Development of Ischaemic Colitis and lactic acidosis
What are the appropriate investigations for MODS?
Monitor Vital Signs
ABG may be necessary to look at hypoaemia, lactic acidosis etc.
