What drugs are used to treat acute gout
NSAIDs like ibuprofen
Colcicine
Corticosteroids
or a combination
For NSAIDs, what is the:
Efficacy
Safety
Suitability
Moderate/good
OK short term, can lead to renal, GI, heart issues if high or long-term
Inexpensive, easy to take, but can have contraindications for GI/cardiac disease
How does colchicine work
It inhibits lymphocyte migration/division
What is the efficacy of colchicine
Very good
How safe is colchicine
Nausea and diarrhoea common, bone marrow suppression and renal failure over long term
How suitable is colchicine
Good option if NSAIDs not an option
How are corticosteroids administered for gout in a single joint
Local injection
How are corticosteroids administered for gout if severe, across many joints
Systemic/tablets ie oral prednisolone
WHy would corticosteroids be prescribed for gout
For patients who cant have NSAIDs or colchicine (older patients with multple comorbidities)
How does allopurinol work
Xanthine oxidase inhibitor, reduces uric acid production
When is allopurinol prescribed
Chronic gout
What is the efficacy and safety of allopurinol
Both good
When is februxostat prescribed
When allopurinol cannot be taken/has bad side effects
