What would you ask about in site?
A
- Where is the pain?
- How many joints are affected?
- Is it symmetrical?

What would you ask about in onset?
A
- Acute = gout.
- Subacute = septic arthritis, RA.
- Chronic = OA.
- Has it been constant since onset?
- If episodic, what is freq/regularity/duration?

What associated symptoms would you ask about?
A
- Stiffness/swelling?
- Erythema?
- Reactive (can’t see/pee/climb tree).
- Systemic temperature.
- Rashes/skin conditions (psoriasis/erythema nodosum).
- Nodules (RA, gout).

What systemic questions would you ask?
A
- Fever.
- Abdo pain/weight loss (systemic symptoms of vasculitis/IBD).
- Dry mouth/gritty eyes.
- Night sweats.

What would you ask about in timing?
A
- RA = early morning stiffness (>60m), joints stiffen after resting, better with activity.
- OA = early morning stiffness <30m, joint pain worse by activity, stiffen up by evening.

What would you ask about in exacerbating/alleviating factors?
A
- Exercise makes worse in mechanical, better in inflammatory.
- NSAIDs can help.

A
- Any particular movements that are painful?
- Any functional issues?

A
- Recent infective episodes?
- Hx of trauma.
- Diabetes (infection predisposition).
- Psoriasis, IBD, coeliac?
- Autoimmune?

What are the risk factors for gout?
A
- Alcohol, renal disease, low dose aspirin, diuretics.

What would you ask about in DH?
A
- Current medication?
- Any OTC and do they work?

A
- Occupation (does the problem affect their employment?)
- Sports/hobbies.
- Home circumstances/ADLs.
- Smoking/alcohol Hx.

What would you ask about in FH?
A
- Hx of RA/OA/psoriasis/gout/IBD or connective tissue/autoimmune diseases.
