Progressive supranuclear palsy
Supranuclear ophthalmoplegia (vertical more than horizontal), axial rigidity, neck extension, impaired postural reflexes, neck extension, inability to look down
corticobasal ganglionic degeneration
apraxia, cortical sensory impairment, alien-limb phenomenon, severe unilateral rigidity, stimulus-sensitive myoclonus
diffuse lewy body disease
early dementia, prominent visual hallucinations, sensitivity to extrapyramidal side effects of antidopaminergic neuroleptic drugs
vascular parkinsonism
lower half parkinsonism with rigidity in the legs and marked gait impairment, corticospinal tract dysfunction, pseudobulbar palsy
multiple system atrophy
early and prominent features of autonomic dysfunction, evidence of corticospinal tract dysfunction, cerebellar signs, stimulus-sensitive myoclonus, vocal cord paresis
stiff-person syndrome
fluctuating and progressive muscle rigidity
- stiffness of axial and trunk muscles spread to proximal limb muscles over time
“TIN MAN SYNDROME’
meige syndrome
focal dystonia characterized by blepharospasm, forceful jaw opening, lip retraction, neck contractions, tongue thrusting
Tx: botulinum toxin
olivopontocerebellar atrophy
progressive cerebellar ataxia, falls no weakness or atrophy
